It is the goal of Habilitation Services to enhance the development of each participant we serve by recognizing and facilitating their maximum potential. All services are based on a person-centered philosophy where each participant is encouraged to assist the team to define their needs and identify the desired supports. Habilitation services offers a fully accessible, warm and supportive atmosphere for people who are 18 years and older who have a disability.
The program encourages relationships and creates a culture that supports, involves, and validates the participant. The program takes into consideration the impairments and limitations of the persons served, promotes improvements and maintenance of their physical and mental health, supports their sense of control and self-determination, and provides a safe environment.
Participants can make new friends, participate in daily activities, and go on community outings all from a safe and monitored environment. The program consists of structured, comprehensive, nonresidential program which provides health services, social work services, recreational activities and personal care services.

Community Integration Service
UCP provides an outreach service to enable its participants to become more involved in the community around them. Called Community Integration Services, the service conducts activities that respond to the needs and desires of UCP participants. The Community Integration Coordinator consults with the participant, their staff and family, and other UCP staff to determine an appropriate approach to working toward meeting participant goals.The service also incorporates a training component, so that participants receive a suitable level of personal skills reinforcement before and concurrent with their community outings. Through their involvement with the service and their community activities, participants have the opportunity to maintain and develop skills and supports, expand their variety of life experiences, feel more a part of the community they live in, and realize the possibilities they can create for themselves. The objective is to build natural supports in the community and foster a greater level of independence in their lives. Participants will experience a greater feeling of control of their lives, and decrease their dependence on paid services.
Some of the activities that have been offered include volunteering at a preschool, walking dogs at the Capital Area Humane Society, and taking a tour of Ohio Stadium. Service expansion will be based on the goals of the participants.
Service Coordination
UCP has three fulltime licensed social workers on staff. They provide comprehensive information and referral services, complete intakes, provide quality assurance and coordinate services/programming for participants of our Adult Day, Supported Living and Employment programs.Nursing
Meals
All participants can enjoy a light breakfast, a healthy lunch and an afternoon snack. Meals are provided through the Child and Adult Care Food Program (CACFP) and provide one third of the recommended daily allowances.In accordance with federal law and U.S. Department of Agriculture policy, this institution is prohibited from discriminating on the basis of race, color, national origin, sex. age, or disability. Fees and Financial Aid
The UCP Adult Day Center is funded in part by Title XX, United Way of Franklin County and various other foundations and grants. If eligible, a licensed social worker will assist you with obtaining financial assistance from one of the resources. If not, private pay options are available.Transportation
Most of the 16 vans in our fleet are modified (handicap accessible), and provide pick up and drop off services at the home and Center, and for planned community trips and area events.
UCP would like to express thanks to the Ohio Department of Transportation for their generous support of our programs. UCP currently has five vehicles purchased through grants from O.D.O.T.
Consumer Council
The Consumer Council is a committee that provides participants a forum to express their views about issues of concern to them, both here at UCP and in the world outside. The monthly meetings are conducted by an elected board of officers (in conjunction with the staff liaison) and are open to all interested participants and staff to attend. Officers are elected in December to serve the following year.The council is a prime vehicle for participants to advocate on their own behalf and offer their input toward the operation of UCP. Topics that are brought up at meetings are presented for discussion and possible action. To ensure that its voice is heard, the council has a direct line to the Executive Director, who attends meetings whenever possible and discusses the Consumer Council meeting minutes at the Directors Meetings.
The council also endeavors to raise funds for various causes. It receives a percentage of proceeds from the vending machines at the center, and it hosts a monthly donut sale. In the past, the council has donated money to a number of recipients: it has donated to agency events; it has given to families in need at Christmas time; it has made funds available for participants without money to go on outings; through the Red Cross, it has donated to the September 11 relief fund, Hurricane George relief fund, and India earthquake relief; and it has purchased and maintained the fish tank in the commons at UCP. The council recently donated funds for families of American military personnel and the Humane Society.
